How do you run Node in production?

What software/process(PM2, Forever, Docker) and hosting (Heroku, DigitalOcean, etc) do you use to run a Node app on a production server?

I love Node. just made my first app and I want to host it on a Apache server. But I can't seem to figure out the conf files and VirtualHosts to get the app running on port 80. I'm also not sure how to keep the app running. is a process manager needed? is docker overkill on a small app? lots of questions sorry! thank you for looking.
