If img has title, do I still need alt?
What do you think about <a role=button>?
- My thought is, this should be considered, if it might be right-click / open-in-new-tab -able.

I think you need to add an alt text whether you have an image title or not.
I haven't used <a role=button> so I really don't have to say anything about it yet. :)
