How to monitor PostgreSQL connections, cache hit ratio and replication lag (and get paged before it hurts)

Most PostgreSQL outages announce themselves an hour early. Connections creep towards max_connections, the buffer cache starts missing, a replica falls behind. Nobody looks, because the dashboard that
