I worked on enterprise apps (uploaded to Apple's AppStore) that offered configuration options through Managed App Config. However, I was not responsible for the upload procedure. But I don't think that during those reviews there were issues / specific requirements raised by Apple for testing those configuration options. As you correctly stated the Managed App Config, essentially a plist, gets deployed through MDM Commands.
Company IT departments often operate an MDM solution and internal mobile apps. Then they push user certificates to enrolled corporate devices via MDM so employees will experience single sign-on when using the internal mobile apps. This is unrelated to Managed App Configuration
