What are some good examples of Form Designs?

If you know good examples of nice form design, let me see them. I'm particular interested in 'small' forms in 'cards' or 'panels' (Bootstrap 3). Could be login or update profile information etc.

I'm looking for something clean and visually appealing so both structure and effects.
