Documentation is really important, but I think it also needs a clear management policy. In my project, I organize documents into four tiers.

Tier 1 contains the canonical documents that affect the entire project. Tier 2 defines development rules and guidelines. Tier 3 contains task-level design documents and PRDs, while Tier 4 is for notes and informal working material.

By separating documents into tiers, it becomes easier to define which source takes priority when documents conflict, and to keep the direction of dependencies between documents clear.
