Starting from the plugin's API Logs page is the right split. No request logged means the problem is on the WordPress side, a logged request with an error means it's Keap, and that one check cuts the search in half. The missing infusionsoft- prefix is a nasty one since the form still submits fine and nothing looks broken to the visitor.
